The chemical structure of REQUIP resembles that of dopamine, and REQUIP mimics the activity of dopamine, with specificity for stimulating dopamine receptors on the postsynaptic neuron.7

These receptors are located on the side of the synapse where the neurotransmitter dopamine is received from an adjoining neuron. The postsynaptic site of activity of REQUIP is important because it bypasses the presynaptic neurons, which are degenerating in PD patients.7


Dopamine Biosynthesis



The dopamine receptors play a vital role in motor control. These receptors need adequate stimulation by dopamine in order to properly direct the body’s movements and flexibility. REQUIP appears to exert its action by stimulating these receptors directly, in much the same way as dopamine.7

The disease is marked by a loss of dopaminergic neurons in the substantia nigra pars compacta, which are normally abundant. These neurons project to the striatum (part of the basal ganglia), where dopamine concentration is reduced by approximately 50% to 75% in people with symptomatic Parkinson's disease.12
